REGISTER HERE FOR THE 2009 PSORIASIS CURE NOW VIDEO CONTEST

Registering for this contest requires you to follow a simple two-step process.

First, upload your video to YouTube and add it to the appropriate Psoriasis Cure Now 2009 Video Contest group: either the Walk group or the Web group. You must become a member of YouTube (membership is free) to upload videos there. (YouTube is not affiliated with us or this contest.)

Next, complete the registration form below. As you will see, you must first upload your video (step one), as we will need the YouTube URL of the video you are submitting. That URL is given to you by YouTube once you upload your video to their site.

Remember to submit this registration form before the filing deadline (October 20, or September 14 if you want to be eligible for the Early Bird prize)!

If you also decide to submit a video to the second track of this contest, please review Rule 8(3) carefully in the Official Rules for the procedure to enter a second video.
